It has been pointed out that the Teacher Help Form in the CM Document is only for Keyboard Teachers. We apologize that we didn't make provision for other disciplines.
Please do not revise the keyboard teacher help form. The form for Strings, Winds and Voice will be posted in a couple of days, in time for you to fill it out and send to CM Chair, Winna Tan. They will be listed separately in the Fliers and Forms section.
